Getting started is quick and easy:

  1. Please fill out our intake form below and submit it. The more detail, the better!

    • Note: Please finish up to the last page and sign and click submit. Please don’t refresh the page as it will reset the entire form!

  2. Once we have received your form, we will contact you to schedule your initial consultation and examination

We don’t share your information. These forms go directly to our staff, even when partially filled, and is stored in a secure database to protect your sensitive health information.

  • Please fill out the appropriate intake form above. Make sure to fill out as much detail as possible so our doctors can assess your case before your first visit.

    Once you submit the form, we will contact you during office hours for your first appointment

  • We will contact you during regular office hours. Please leave your best available phone number and email address on our intake form and we will contact you as soon as we can to book your initial assessment

  • No problem! Make sure to fill out the first page with your contact information and we will contact you to book your appointment first.

  • Our office is on 3961 Hastings St. Unit 101 , Burnaby, BC.

    There is ample street parking that is free and parking lots with free parking along the side of MacDonald and Ingleton Ave.

    Please note
    Parking across the street on the south side of Hastings St. is prohibited after 3PM, and cars will be towed. The North side (where the clinic is) does not have this restriction

  • Your first visit will be about an hour long, which we will take:

    • A physical exam of your spine, muscles and nerve system

    • Scans of your spine with state of the art technology

    • A thorough health history

    • Structural chiropractic x-rays if necessary

    Afterwards the doctors will review your results at your next appointment with a comprehensive plan to get your health back!

  • Anyone under the age of 18 will have to fill out the Child Intake form.
